| Description | Applications for Tax Identification Numbers shall be accompanied by the following supporting documents: (i) Copy of identification card, family book, certificate of residence or passport of the business owner or an individual or legal entity who is authorized by the business owner to make an agreement or conduct legal activity on behalf of them; (ii) A map of the business location certified by local authorities, telephone number, mobile number, fax number, email that are contactable; (iii) Copy of certificate of concession, business license, certificates from related agencies and other certificates, if applicable |
